• Accident Medical and Dental - $5,000 accident medical and dental coverage for your entire family.  Coverage is valid at any hospital or physician's office.
   
• Hospital Indemnity - 0 days elimination, plan pays $150 per day for up to 31 days for any qualified accident.  Covers all family members.
   
• Accidental Death or Dismemberment - $10,000 benefit covers all family members.  Group policy is issued by a major insurance company.  10% bonus benefit if involved in a motor vehicle accident while wearing a seat belt.
   
• Eligibility - All dues paying members, under age 70, and dependent children, under age 19, living at home.  All eligible applicants accepted.
   
• Major Insurance Carrier - policy is underwritten by a major insurance carrier.
